Predicting HIV Care Engagement Attrition: Who is at Risk and How Health Systems Shape the Care Journey

Retention in HIV medical care is critical for achieving and maintaining viral suppression, improving health outcomes, and reducing transmission risk. For people living with HIV, being “in care” is defined as consistent engagement with healthcare services, including regular clinical visits, laboratory monitoring, and adherence to antiretroviral therapy. Despite advances in HIV care and treatment, loss-to-follow-up (LTFU) remains a persistent challenge. Patients who disengage from care are at increased risk for HIV-related morbidity and mortality and may unknowingly transmit the virus to others. Understanding where and when disengagement occurs is essential for developing targeted interventions. This study aims to assess retention in HIV care among patients at the Holloway Community Program, characterize patterns of LTFU, and identify missed opportunities for re-engagement. By leveraging electronic health records, billing claims and pharmacy data across multiple Ryan White-funded clinics within ChristianaCare, we seek to inform data-driven strategies to optimize long-term care engagement.

Meet the Speaker
Dr. Duffus is board certified in Infectious Diseases and is the Clinical Director for the Holloway HIV Community Program, School-based Health Centers and Juvenile Justice Health. Prior to this he had roles in the pharmaceutical industry and government (Federal, domestic and international) and state level. This includes as Executive Director for Global HIV Prevention at Gilead Sciences; acting State Epidemiologist for the Illinois Department of Public Health; Senior Medical Advisor to CDC- South Africa PEPFAR Care and Treatment Branch and as the Associate Director for Health Equity at CDC’s National Center for HIV/AIDS, Viral Hepatitis, STD, and TB Prevention.
